Overview

Description

Center for Life Management (CLM) provides the area's most comprehensive array of mental health services. Our Collaborative Care Program integrates mental, physical and behavioral health needs with a multi-disciplinary team of internal CLM staff and external partners to support individuals with co-occurring mental health needs and intellectual disabilities and/or traumatic brain injury. The Collaborative Care Community Support Counselor provides outpatient, community-based support to help clients improve symptoms and behaviors that interfere with daily functioning and personal wellness goals.

This is a full-time, 35-hour-per-week position.

Functional Area

s and Description of Duties
  • Provide psychoeducation using the evidence-based practice of Illness Management and Recovery (IMR) to support goal and coping skill development.
  • Provide counseling and Functional Support Services (FSS) in home and community-based settings as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.
  • Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.
  • Provide Family Training and Counseling (FTC) services to educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals.
  • Provide Medication Training and Support (MTS) services, as needed to educate, problem solve and develop strategies to incorporate prescribed medication into daily routine.
  • Provide Crisis Intervention (CI) services, as needed, when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.
  • Adhere to and uphold the practices specific to the Continuum of Collaborative Care Model to support the authenticity of program implementation.
  • Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach for continuity of client care.
  • Provide timely documentation on each service delivered.
  • Provide set hours of billable service each week (eligible for team-based incentive pay upon exceeding service expectation).
  • Engage in appropriate consultation meetings and committees, both internally and externally, as determined by supervisor.
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients.
  • Execute all required services and documentation in accordance with agency policies and federal/state standards.
  • Maintain professional memberships, licenses, accreditation, certifications, etc.
  • Represent CLM in a professional and appropriate manner in all settings.
  • Perform all other duties as assigned by supervisor.
