We’re looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. A successful candidate is highly empathetic, can juggle the needs of our clients, programs, and business, and is eager to make a lasting impact. Comfort working with new systems and tools is an advantage. A huge plus if home modification work is part of your experience.

  Contracted, minimum of 10 hours weekly availability

  In‑Home Visits

• Conduct in‑home assessments and functional evaluations
• Deliver personalized, evidence‑based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility techniques
• Engage clients and families in collaborative care planning
• Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives
• Maintain accurate and timely documentation through our electronic system

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver’s license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team

• Contractors are paid on a monthly basis, within 2 weeks of the previous month’s end.
• We pay per completed evaluation, which includes:
  • Travel to/from client’s home
  • The evaluation itself
  • Documentation
• If needed, we will pay additionally for extended travel – we will always discuss and approve ahead of time.
