OverviewThis role is responsible for primary ownership and oversight of a specified panel of members with varying health needs. The incumbent assesses members’ health management needs using data and analytics, and identifies appropriate clinical/lifestyle interventions. The role involves outreach to members enrolled in disease management, personalized care planning, behavior change encouragement, barrier identification, care coordination, and resource referral, primarily via telephonic, digital, or virtual engagement to improve health outcomes.
EssentialResponsibilities
- Maintain knowledge of evidence‑based interventions and industry standards to deliver high‑quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, and maternal health.
- Oversee the assigned member panel by performing ongoing assessment of health management needs and triaging members to appropriate resources.
- Create care plans addressing members’ identified needs, remove barriers to care, identify internal and external resources, and conduct activities to improve health outcomes; care plans include both short‑ and long‑term goals and a planned schedule of contacts for reassessment.
- Demonstrate clinical outcomes and cost savings based on wellness interventions across age bands, regions, and products.
- Ensure all activities are documented and conducted in compliance with business process, regulatory, professional scope, and accreditation standards.
- Participate in work groups focused on improving the efficiency and member‑centric focus of wellness coaching services, including development and implementation of new programs and pilots.

- Master’s Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or a related field OR relevant experience and/or education in lieu of a master’s degree.
- 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness‑model environment (i.e., health plan/insurance, disease or condition management, or acute care setting).

- Registered Dietician
- Certified Diabetes Educator (CDE)
- Exercise Physiology Certification
- Certified Lactation Counselor
- National Diabetes Prevention Program Lifestyle Coach
- Certified Health and Wellness Coach (CHWC)
- Familiarity with Highmark’s preventative health services programs and services.
- Effective presentation and strong relationship‑building skills.
- Proficiency with Microsoft Teams and basic computer applications.
- Comprehensive knowledge of current health education practices.
- Knowledge of Prochaska’s Transtheoretical Stage of Change model.
- Team‑oriented, flexible, and self‑directed with moderate autonomy.
- Excellent organizational, time‑management, and project‑management skills.
