Full‑time | Monday – Friday | 8 am – 5 pm with optional 4/10 work schedule

We are currently seeking a compassionate, team‑oriented Care Team Nurse (LPN/RN) to support patients across two clinic locations.

YOUR ROLE

Provide practice‑management support to the clinical team, embracing the Patient‑Centered Primary Care Home (PCPCH) by recognizing the patient as the central focus. The Care Team Nurse, in collaboration with other health‑care professionals, addresses patients' wellness, acute illness, chronic disease, disability, and end‑of‑life needs. Practice includes telephone advice for minor acute problems; patient education on self‑care; ongoing management of stable chronic disease;

monitoring of acutely ill patients during routine office visits; and performance of procedures within the scope of professional nursing practice. The nurse oversees the provider’s assigned patient panel by directing and facilitating care and using the nursing process: assessment, planning, intervention, and evaluation.

SPECIFIC DUTIES
  • Participates in the ongoing management of patients with stable chronic diseases, as designated by and in consultation with the Primary Care Provider (PCP). Communicates with patients in person and over the telephone regarding the management of their chronic disease and coordinated treatment plans or services, including follow‑up as needed.
  • Manages triage, intake, and monitoring of patients with acute medical conditions. Monitors patients in the absence of their PCP, providing necessary nursing intervention when appropriate.
  • Under PCP direction, provides patient education on self‑care, including medication administration, nebulizer use, inhaler technique, diabetes management, wound care, and self‑injection. Explains treatments and procedures to patients.
  • Assists providers with patient examinations and procedures (e.g., minor surgery, casting, nebulizer treatments, spirometry, EKG) as well as medication administration, based on clinical needs and scope of nursing practice.
  • Accurately observes, analyzes, and interprets data both verbal and written, communicating clinical findings. Recognizes abnormal and critical findings and takes appropriate action, including notifying the provider.
QUALIFICATIONS
  • Current Oregon RN or LPN license.
  • Valid Basic Life Support for Healthcare Providers (BLS) certification in line with AHA CPR/ECC guidelines covering cardiorespiratory emergencies for adults, children, and infants.
  • Prior clinic experience is preferred.
  • Ability to exercise independent judgment and decision‑making.
  • Ability to work effectively in a team setting.
  • Capacity to prioritize competing responsibilities under pressure.
  • Problem‑solving skills and personal flexibility to thrive in a dynamic environment with frequent interruptions.
  • Ability to manage high volumes of patients in short spans while handling unpredictable issues.
