Position OverviewAs an LVN/LPN in a skilled nursing setting, you will provide direct patient care, support interdisciplinary teams, and ensure residents receive safe, compassionate, and efficient care.
Responsibilities- Administer medications and treatments as prescribed
- Monitor and document patient conditions and vital signs
- Provide wound care and other skilled nursing interventions
- Collaborate with RNs, physicians, and care teams
- Assist with admissions, discharges, and care planning
- Ensure compliance with facility policies and state regulations
- Deliver compassionate care to residents and their families
- Active LVN/LPN license in good standing
- Skilled nursing or long-term care experience preferred
- Current CPR/BLS certification
- Strong clinical and documentation skills
- Ability to work in a fast-paced environment
