Physical Therapist; PT; Home Health

What You’ll Do

• Conduct comprehensive initial evaluations and assessments and promptly report findings to the physician/provider and Clinical Manager
• Develop, implement, and update individualized plans of care based on patient needs and physician orders
• Perform skilled physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests in the home setting
• Complete initial evaluations, treatment documentation, and re-evaluations every thirty (30) days; update plans of care as needed
• Complete comprehensive assessments and OASIS documentation accurately and on a timely basis, as applicable
• Observe, document, and report patient response, progress, and any significant changes to the physician/provider and care team
• Communicate and collaborate with physicians, providers, and interdisciplinary team members to ensure coordinated, high-quality care
• Provide education and support to patients and their families to promote safety, independence, and long-term success
• Assist with orientation, supervision, and consultation for Physical Therapy Assistants, as assigned

• Master's degree in Physical Therapy
• Active Physical Therapy license in good standing in the state of California
• At least one year of experience working as a Physical Therapist
• Must have a vehicle, a valid current driver’s license, and automobile insurance
• Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner
• Home Health and Home Care Home Base documentation experience
