Work Days &

Hours:

Contingent Position

• Any day Monday - Friday, set own hours
• Paid travel time, paid mileage and pay per visit rates
• 3-4 admissions/week
• On-Call Rotation:
  Yes
• 1 summer holiday, 1 winter holiday and 1 weekend a month

What you will do as a Physical Therapist with Shared Services Home Health Care

• Review request for physical therapy and physicians orders.
• Interview client to determine type of treatment and schedule therapy.
• Evaluate type of therapy most desirable after consulting with physician.
• Assist clients in ambulatory training as necessary.
• Recommend modifications and changes in the clients therapy treatment as indicated.
• Ensure that all therapist notes are informative and descriptive of the care provided and of the clients response to the care.
• Select rehabilitative and therapeutic activities and exercises based on medical and social history of clients.

What it takes to join our team

• Doctoral degree in Physical Therapy from an accredited college or program
• Must possess an active license to practice as a Physical Therapist in the State of Michigan in good standing
• 1 year of homecare experience preferred
• Experience with Wellsky/Kinnser and Forcura preferred
