• Provide skilled nursing care and assessments to patients in their homes.
• Develop, implement, and evaluate individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and communicate any changes to the healthcare team.
• Educate patients and their families on disease management and health maintenance.
• Maintain accurate and timely documentation of patient care.
• Coordinate with interdisciplinary team members to ensure continuity of care.

• Valid Registered Nurse (RN) license in the state of Michigan.
• Excellent clinical assessment and critical thinking skills.
• Strong interpersonal and communication abilities.
• Ability to work independently and manage time effectively.
• Reliable transportation to travel to patient homes within the designated area.

• Competitive hourly wage of $14.00 - $16.00, paid bi-weekly.
