- Initiate the admissions process for hospice patients by completing the initial comprehensive assessment, coordinating initial services provided by the Interdisciplinary Team, providing medical care, and developing the initial plan of care based on the patient/caregiver/family needs and goals.
- Be responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
- Utilize equipment and technology skillfully and is technically competent in nursing procedures
- Oversee and/or participates in safe medication handling and practices
- Educate patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment, and ensures consent and payer forms are completed
- Act as a patient advocate; provides support, information and direct care
- Triage phone calls to identify patient/family needs and notifies Admission Services of referrals
- Oversee and delegate assignments of CMTs, LPNs, Hospice Aides, students, and volunteers on shift
- Participate in weekend, holiday, on-call and/or evening/night rotations
- Maintain appropriate licenses and adheres to continuing education obligations
- Have the ability to cover different areas of hospice care to assure organizational needs are met
- Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
- BSN from an accredited college preferred
- CHPN certification expected within two (2) years of employment

In this role, you will:
