Undertake holistic, strengths-based OT assessments with adults with complex physical, emotional, or social needs. Design and deliver innovative reablement plans to help customers achieve their goals and reduce dependence on long-term support. Lead on complex risk assessments, moving and handling assessments, and mental capacity considerations. Recommend and coordinate minor and major home adaptations. Work collaboratively with families, carers, health partners, support workers, and voluntary sector organisations.
Provide professional supervision and support to other Occupational Therapists and Customer Coordinators. Maintain accurate, timely clinical records and contribute to service development. Uphold statutory responsibilities including safeguarding, legal frameworks, and professional standards.

Essential: A recognised Occupational Therapy qualification (Diploma or Degree). HCPC registration as an Occupational Therapist. At least 2 years post-qualification experience . Strong knowledge of Health, Social Care, and Housing legislation. Experience working with adults, including complex cases. Experience delivering adaptations via the Disabled Facilities Grant process. Proficiency in moving and handling assessments. Excellent communication, analytical, and interpersonal skills. Ability to prioritise and manage a varied caseload.
Confidence in supervision, professional challenge, and evidence-informed practice. Competence with ICT and accurate record-keeping. Desirable:
Experience in social care, reablement, or intermediate care. Supervising OT students or staff. Safeguarding investigating officer experience. Training delivery, especially in moving and handling. Additional Requirements Ability to travel across Wiltshire to visit customers in their homes and community settings. Occasional work outside standard hours (up to 10% of contracted hours) to meet service needs. Why join us?
