Join a dedicated service that helps older people, including those living with dementia, regain their independence following a hospital stay. As a Discharge Flow Coordinator
, you'll play a key role in ensuring people receive the right support at the right time, working alongside a skilled multidisciplinary team to deliver positive outcomes and smooth transitions of care.

• Coordinate individuals' journeys through Short-Term Services.
