Supporting High-Quality Diabetes Care Across the Community
This is an exciting opportunity for an experienced nurse with a passion for diabetes care to join our community-based specialist team. The role focuses on delivering expert assessment, treatment, education, and support to patients with diabetes, their families, and carers.
The successful candidate will work within a multidisciplinary team while exercising a high degree of professional autonomy in the management of patients with diabetes. They will contribute to the delivery of safe, effective, and patient-centred care, support service improvement activities, and work collaboratively with colleagues across Wiltshire to ensure consistently high standards of practice in line with local and national guidance.

- Deliver specialist diabetes nursing care to patients across community and primary care settings.
- Undertake complex clinical assessments, identifying physical, psychological, social, and cultural factors that may impact care.
- Initiate, review, and evaluate diabetes treatment plans in accordance with local and national guidance.
- Work autonomously while contributing effectively within a multidisciplinary team environment.
- Provide specialist advice, education, and support to patients, carers, and healthcare colleagues.
- Contribute to clinical governance, service development, audit, and quality improvement activities.
- Support the development of standards, protocols, and safe clinical practice across the service.
- Maintain high-quality clinical documentation and participate in team and professional network meetings.
- First Level Registered Nurse.
- Evidence of post-registration development in diabetes nursing.
- Significant post-registration experience within the speciality of diabetes.
- Experience supporting patients with diabetes in acute, community, or primary care settings.
