Join Elara Caring as a Hospice Nurse Practitioner and help deliver high‑quality home‑based care to more than 60,000 patients each day.

• Conduct routine and emergency face‑to‑face recertification visits for patients in their third benefit period or beyond, including admissions, transfers, and extended length of stay.
• Record observations, treatments, and other pertinent information.
• Communicate findings to the Medical Director, attending physicians, branch leadership, and IDG.
• Provide physical and emotional care to support patients and families during periods of crisis.
• Triage consults by patient acuity.
• Perform bedside palliative care with patient/family education.
• Ensure activities align with Elara Caring’s mission and leadership expectations.
• Prevent unwanted hospitalizations by meeting patient and family needs at home.
• Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
• Comply with applicable laws and regulations, including collaborative agreements.
• Complete accurate billing and coding activities for all patient encounters per CMS guidelines.
• Provide training and continuing education for staff.
• Assist in development of clinical practice guidelines and standards to support quality palliative care.
• Adhere to confidentiality practices regarding patients, families, staff, and the organization.
• Collaborate and communicate with Medical Director, attending physician, branch leadership, and IDG.
• Perform other duties and projects as assigned.

• Master’s degree in nursing and certification through a national Nurse Practitioner licensing board.
• 1+ years of clinical nursing experience.
• 1+ year of post‑graduate clinical nursing experience in hospice and palliative care preferred.
• Prior experience in family medicine, geriatrics, and psychiatry preferred.
• Current licensure in the state of practice as a Nurse Practitioner and maintenance of such licensure in accordance with applicable laws and regulations.
• Maintain active DEA certification if the state allows prescription of controlled substances by APPs.
• 50% travel required.
• Ability to sit, stand, bend, lift, and move intermittently; lift 50–100 lbs.
• Valid driver’s license, insurance, and reliable transportation to perform job tasks.
