Age Care is seeking an experienced LPN to contribute to our inter-professional team focused on person-centered care. Your responsibilities will include ensuring resident care plans are accurate, providing direct nursing care, and effectively managing documentation. Key Responsibilities:

• Work within an inter-professional team framework

• Conduct and update clinical assessments for residents

• Implement and review effectiveness of care/service plans

• Organize resident care conferences when needed

• Provide medication and direct care while documenting accurately

Requirements:

• Practical Nurse Diploma with good standing registration

• At least 3 years of experience in senior care

• CPR certification and proficiency in computer applications

• Skills in leadership, coaching, and mentorship
