Job SummaryThe Practice and Patient Care Coordinator (PPCC) plays a key role in the practice by working with practice teams and patients in building systems, (re) designing processes, supporting the financial care coordination and collaborating across the clinic work groups to improve the quality, equity and outcomes of care at the University of Utah. In addition, the PPCC will provide care coordination, hands‑on as well as facilitating the provision of care coordination activities by member of the practice team.
Responsibilities- Coordinates complex schedules of patients, such as radiology, cardiology testing, lab work, and any necessary testing pre and post-surgery
- Organizes required diagnostic screenings
- Assists in the coordination of outside lab work, gathering results, and documentation
- Schedule and coordinates patients’ surgeries, including any necessary procedures needed prior
- Coordinate and navigate providers surgical and clinical schedules
- Collaboration with other departments for integrated patient care
- Coordinates with other university of Utah departments and insurances to ensure prior authorizations are obtained
- Acts as a liaison with patients, outside healthcare professionals, and ancillary hospital departments
- Provides follow-up and support as needed with patient request of the physician and care team
- Assist patients and family members in navigating the healthcare system, including access to health insurance, financial aid applications and eligibility, explanation of insurance benefits, resources for uninsured or low-income patients. Triages and manages calls by providing clinical information to patients, referring physicians, lab and insurance carriers
- Processes and distributes all transcribed notes, letters, and reports
- Distributes written communications according to department protocol
- Maintains current lists of patients for various phases of treatment. Creates new patient charts ensuring that the appropriate documentation is included. Maintains filing on all patient charts
- Acts as the first point of contact for the department by answering the phone, greeting, and directing patients
- Collaborate and work as a team to improve patient care and education in order to provide an exceptional experience
- Bachelor's degree or equivalency (one year of education can be substituted for two years of related work experience) in public health, health education, health promotion, management, behavior science, social science, social work or nursing with 2 years relevant work experience
- Medical Assistant with State of Utah, National Certification, or equivalency and 5 years of clinical experience
- Demonstrated ability to lead as well as function effectively as a member of a team and successfully promote change
- Strong analytical, organization and leadership skills
- PC/Windows literate with working knowledge of Outlook, Word and Excel
- Demonstrated human relation and effective communication skills
- Patient-sensitve position
- Working knowledge of quality improvement and project management methodologies, tools and techniques and principles of change management
- Demonstrated ability to effectively facilitate and mentor project teams preferably using QI methods (i.e., PDSA)
- Familiarity with electronic medical record (EMR) applications
- Patient-sensitive and must fulfill all associated requirements, including immunization according to CDC standards and hospital policy
