The Road Home was founded in 1923 and has been a leader in ending homelessness for over 100 years. We provide emergency shelters, supportive services, and housing-focused rental assistance that helps individuals and families step out of homelessness and back into the community. The Road Home seeks employees that have a deep commitment to the mission of serving adults and families that are experiencing homelessness.
The House 20 Case Manager works with individuals that are homeless and have frequent interactions with emergency services, including police, fire and ambulance services in Salt Lake City. The House 20 Case Manager will work alongside a second CM to provide outreach services to eligible clients and will work to engage with them and support them as they move towards housing.
The objective of this position is to provide Housing First and client driven case management services to homeless and formerly homeless individuals participating the House 20 program. Above all else, Case Managers seek to assist clients in achieving stability in housing by connecting them with the resources they need to become more self-sufficient.
