We are a locally owned and operated hospice and home health agency celebrating 27 years of high-quality, patient-focused care. We are looking for a qualified RN Case Manager who shares our values and desires to assist in delivering excellent care to our community.

- Provide skilled nursing care, including wound care, catheter care, venipuncture, observation, medication compliance, teaching, and hospice assessments.
- Create patient-centered goals with patients, family members, and providers.
- Assess, supervise, and direct patient care.
- Develop plans of treatment.
- Ensure chart compliance with state regulations.
- Coordinate patient care with physicians and other providers.
- Licensed RN by the State of Utah.
- Current Utah driver’s license and automobile insurance.
- CPR certification.
- Minimum of 1 year nursing experience.
- Hospice and home health experience strongly preferred.
Associate degree
Required Experience1 year of nursing experience
Required License or CertificationRegistered Nurse (RN)
