Schedule:

Full Time, Part Time, PRN
• 10‑12 hours per shift (Days Only). Available Weekdays or Weekends

Sign‑on bonus available for full‑time and part‑time RN/LPN.

As a Pediatric Home Health Nurse, you will provide specialized nursing care to pediatric patients in the comfort of their homes. You will promote and maintain the health and well‑being of patients with chronic illnesses or disabilities, working closely with families, caregivers, and other healthcare professionals to ensure patient‑centered, compassionate, and high‑quality care.

• Work under the supervision of a Pediatric RN Case Manager.
• Provide direct patient care in accordance with state Nurse Practice Act and agency policies.
• Carry out the plan of care.
• Educate and counsel patients and families on medication, treatment regimens, and self‑care techniques.
• Document care accurately and submit reports per Harmony Policy and regulations.
• Evaluate outcomes, inform physicians and staff of changes in client needs.
• Comply with HIPAA and maintain patient confidentiality.
• Demonstrate effective time management and organizational skills.
• Communicate the mission, ethics, and goals of Harmony Home Health and Hospice.

• Graduate of an accredited school of nursing.
• Current Utah RN or LPN license.
• Able to work with minimal supervision.
• Reliable automobile and valid driver’s license.
• Pass a criminal background check and drug screen upon hire.
• Current TB test, CPR card, and automobile insurance.
