Your day-to-day as a Nurse Care Manager - Medically Vulnerable Population

As a Full-Time MVP Nurse Care Manager at Fourth Street Clinic, you'll play a pivotal role in enhancing patient care within a respite shelter setting, ensuring individuals have access to essential services.

Your key responsibilities will encompass conducting patient intakes and assessments, crafting comprehensive respite care plans, and providing vital triage and wound care. You'll also support chronic disease management and medication education while expertly coordinating lab work, immunizations, and referrals.

Additionally, you'll facilitate follow-ups for both medical and behavioral health needs, ensuring that each patient receives holistic care tailored to their unique circumstances. Your contributions will not only improve individual health outcomes but also strengthen the community we serve!

Requirements for this MVP Nurse Care Manager job

To thrive as a Full-Time MVP Nurse Care Manager at Fourth Street Clinic, you'll need a blend of essential skills and qualifications. First and foremost, a graduation from an accredited nursing program and an active RN licensure in good standing with DOPL are must-haves. Your strong assessment and organizational skills will enable you to effectively manage patient care in a fast-paced environment, while multitasking will ensure you can juggle diverse responsibilities with ease.

Familiarity with wound care is crucial, along with the ability to prioritize tasks using triage principles. Additionally, maintaining adherence to HIPAA regulations and FSC safety standards will be central to your role. Finally, possessing ACLS/BLS certification upon hire or within 90 days will further enhance your ability to provide outstanding patient care in our supportive environment!
