Overview
First Step House is hiring a full-time PROUD case manager in Salt Lake City, Utah. The PROUD case manager serves clients in recovery from opiate use disorder to promote financial self-sufficiency, housing stability and long-term recovery. The goals of this program are to provide housing case management services as clients transition from residential treatment to a sober living setting, develop individualized service plans that focus on finding permanent housing, and connect clients with community resources and sober supports.
The PROUD case manager assists recovery residence teams with client check-ins and daily facility checks, completes individualized case plans with clients, helps clients find permanent housing options in the community, and provides additional case management services for up to two years to help them remain stable in permanent housing.
Salary: $46,353.60 per year. Pay rate and schedule are described as part of the role.
Responsibilities- Daily check-ins and on-site residence checks
- Address tenant concerns when the Recovery Residence Manager is unavailable
- Perform needs assessments with clients
- Develop individual case management plans with each client and meet regularly to review progress
- Collaborate with outpatient counselors, case workers, and outside agencies to coordinate services
- Perform regular individual case-management sessions
- Collaborate with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
- Use an internal tracking system to ensure proper, accurate, and timely billing
- Use the Utah Web Infrastructure for Treatment Services (UWITS) database to enter case-management notes, client information updates, billing information and other relevant client information
- Case management experience is preferred
- Experience in the substance use/recovery field is preferred
- A minimum of a Bachelor s degree in social work or related field is preferred
