Transitional Housing Case Manager
Provide trauma-informed crisis counseling, advocacy, and culturally competent case management services for individuals and families in the Kathleen Robison Huntsman Apartments (KRH) transitional housing programs at the YWCA Utah.
Essential Job Functions:
- Provide trauma-informed crisis counseling, advocacy, and coaching to women, men and families in crisis due to domestic and/or sexual violence.
- Support each program participant in achieving their plan with courteous, timely and meaningful help.
- Provide case management services, including but not limited to: assessing for danger, safety planning, goal setting, and connection to internal and external supports and resources.
- Support participants with maintaining housing stability, including direct assistance with applying for rental assistance as needed and proactively addressing barriers.
- Assist with family and child-related needs in collaboration with adult participants.
- Proactively offer and arrange interpretation services as necessary.
- Advocate for access to services. Coordinate with resources and problem-solve barriers to access as they arise.
- Provide assistance to the Property Manager and residential advocates in addressing issues that impact housing stability, participant safety, and program functioning.
- Regularly communicate with community resources to improve coordination of services and strengthen referral networks.
- Facilitate and/or co-facilitate participant meetings and groups as requested. Promote engagement and meaningful participation; share participant feedback and advocate for their self-identified needs.
- Ensure safety of campus by monitoring security and intervening in crises as needed.
- Complete all records and documentation in a timely manner, including but not limited to: record intake, review, and exit assessments, enter case plans within 7 days of program entry, complete case notes for all meetings and participant interactions, track referrals and outcomes, maintain accurate, updated case files, assist with documents necessary for section 42 compliance, obtaining signed releases of information for coordination of services.

- Maintain all certifications and licensing related to role, program licensing, and grant standards including but not limited to:
Case Management Certification, CPR, First Aid, etc. - Attend all required staff meetings, individual and/or group supervision, agency-wide staff and other trainings, as well as any other meetings and activities as requested.
