Job summary

We are looking to recruit a dedicated and experienced Nurse Practitioner to join our friendly and supportive Primary Care multi‑disciplinary team on a full‑time or part‑time basis. This is an excellent opportunity for a clinician who is passionate about delivering high‑quality, patient‑centred primary care within a supportive multidisciplinary team.

Main duties of the job

As a Nurse Practitioner, you will provide autonomous clinical care to patients across a broad range of acute and chronic conditions. You will assess, diagnose, treat, prescribe, and refer patients as appropriate, helping to improve access to clinical services, contributing to the overall health and wellbeing of our local patient population. The post holder may be required to carry out home visits and must have access to a vehicle.

About us

East Cleveland PCN consists of 5 member practices serving a patient population of approximately 42,000. The PCN team works with local practices to provide and develop efficient and effective services with patients at the centre. All practices use Systm One clinical system.

The successful applicant will be based at Hillside Practice, Skelton, but may be expected to work in any of our core member practices to support with service delivery.

  • Conduct comprehensive patient assessments and clinical examinations
  • Diagnose and manage acute and long‑term health conditions
  • Independently prescribe medications within your scope of practice
  • Order, interpret, and act upon diagnostic tests and investigations
  • Provide health promotion, disease prevention, and lifestyle advice; teaching and advising patients and their families on how to manage their condition
  • Refer patients to secondary care and specialist services as appropriate
  • Maintain accurate and timely patient records
  • Work within national and local protocols where these exist
  • Work within your own professional boundaries, whilst working at an enhanced level of practice as described in the Primary Care & General Practice Nursing Career & Core Capabilities Framework
  • Work collaboratively with and provide support to other members of the extensive multidisciplinary team. Where appropriate delegate work to other MDT members within their competencies
  • Participate in clinical governance, audit and quality improvement activities
  • Where appropriate provide supervision to trainee clinicians
  • Participate in and/or lead formal training/mentoring to assist the promoting best practice in primary care
  • Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care where appropriate
  • Advise and support on the development and delivery of work streams relating to area of expertise as part of the PCNs strategy, where appropriate
  • Contribute to the delivery of local policy, procedures, and protocols for the PCN within your skill set.
Person Specification – Qualifications – Essential
  • Registered Nurse with current registration with the relevant professional body (NMC).
  • Clinical skills Level 7 or above relevant to their area of advanced practice.
  • Nurse Independent/ supplementary prescriber Level 6 or 7 (non‑medical prescribing).
  • Experience working in primary care or general practice.
  • Excellent communication, organisational and decision‑making skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Experience in chronic disease management.
  • Knowledge of UK primary care systems and pathway.
  • Full UK Driving Licence: required for home visits.
  • Demonstrate strong analytical and problem‑solving skills.
  • Mentorship certificate/Practice assessor training or demonstratable equivalent level.
  • Experience using electronic patient record systems, eg Systm One.
