Job summary

An exciting opportunity has arisen for an experienced Nurse Practitioner to join our clinical team at Huntcliff Surgery for 22.5 hours per week.

Based in the Victorian seaside town of Saltburn by the Sea, Huntcliff is a busy, forward thinking training practice caring for a growing population of approximately 10,000 patients.

Main duties of the job

Working as an autonomous practitioner, you will use advanced clinical assessment and decision making skills to assess, diagnose and manage patients presenting with a range of acute and chronic conditions, both within the practice and through domiciliary visits where clinically appropriate.

You will work collaboratively with our GPs and wider multidisciplinary team, independently prescribing within your scope of practice, interpreting investigations and referring appropriately where further specialist care is required.

The role will include health promotion, disease prevention and chronic disease management, alongside opportunities to contribute to clinical governance, audit, quality improvement and the review and development of clinical policies and pathways.

Job responsibilities Key Clinical Responsibilities
  • To provide a first point of contact for patients with undifferentiated and undiagnosed health needs.
  • Provide domiciliary visits
  • Using advanced assessment skills of history taking and clinical examination, make a diagnosis and initiate an appropriate plan of care, including referral to other health and social care professionals as necessary.
  • Provide ongoing education and health promotion in relation to primary prevention/secondary prevention.
  • Organise and manage own patient caseload.
  • Request, interpret and evaluate diagnostic tests to assess patients' clinical problems or monitor progress.
  • Manage patients with long term conditions, including the development, implementation and evaluation of individualised treatment plans.
  • To provide a holistic nursing assessment drawing on knowledge about the wider determinants of health and ill health.
  • Independently prescribe medication within own scope of practice and undertake medication reviews in accordance with current guidance.
  • Participate in the development and implementation of Practice/PCN and wider system clinical pathways and guidelines where appropriate.
  • Provide clinical care within own scope of practice and recognise, assess and appropriately refer patients presenting with mental health or safeguarding needs.
