Job Purpose The Community Navigator supports disabled people, unpaid carers and people with long-term conditions to access services, opportunities, and community resources that improve independence, wellbeing, and social inclusion. The role focuses on person centred support, empowerment, and reducing barriers that prevent people from living full and connected lives.

Key Responsibilities
  • 1. Person-Centred Support Work one-to-one with people to identify goals, needs, and priorities

    Practical, hands-on assistance to support people with paperwork, attend appointments or a support group, online applications and build confidence to support independence.

    Provide practical guidance to access services, benefits, community activities, and supports
  • 2. Navigation & Advocacy Help people understand and navigate health, social care, housing, welfare, and community supports.

    Support people to self-advocate and build confidence Signpost and refer to appropriate services and opportunities
  • 3. Community Connection Build knowledge of local resources, groups, and supports

    Develop relationships with community organisations and partners

    Promote inclusive opportunities that reduce isolation and increase participation
  • 4. Partnership Working Liaise with statutory, third sector, and community providers

    Attend multi-agency meetings where appropriate

    Contribute to collaborative approaches that improve outcomes for individuals
  • 5. Monitoring & Reporting Maintain accurate, confidential case records

    Record outcomes and impact using agreed systems

    Contribute to evaluation, reporting, and service improvement
  • 6. Safeguarding & Professional Practice Follow safeguarding, confidentiality, and data protection procedures

    Work within organisational policies and professional boundaries

    Participate in supervision, training, and reflective practice
