This position involves independently delivering professional counseling services to patients with chemical dependency and psychiatric issues in a clinical setting.
Role OverviewThis position involves independently delivering professional counseling services to patients with chemical dependency and psychiatric issues in a clinical setting.
Key Responsibilities- Identify patients and families with complex psychosocial needs requiring intervention and document pertinent information for the healthcare team.
- Conduct comprehensive assessments within 72 hours of admission, including medical, social, emotional, financial, and support system evaluations.
- Ensure assessments conducted by licensed professionals are signed and approved by a master social worker.
- Develop discharge and care management plans collaboratively with patients, healthcare team, and payers, and communicate adjustments based on patient progress.
- Advocate for patients by ensuring understanding of medical care options and documenting case management activities.
- Coordinate interdisciplinary services, including referrals, discharge planning, and teaching, to ensure timely, patient-centered care.
- Provide therapeutic individual and group counseling, monitor patient progress, and review ongoing care plans for consistency with clinical needs.
- Arrange for post-discharge services such as prescriptions, appointments, DME, and referrals, ensuring documentation is complete.
- Maintain confidentiality, adhere to ethical standards, and demonstrate competency in legal procedures like emergency detention and guardianship processes.
- Handle insurance authorizations, hospital policies, and safety documentation, and participate in meetings, trainings, and other duties as assigned.
- Experience in social work, counseling, or case management within psychiatric or chemical dependency settings.
- Knowledge of community resources, legal, and insurance processes relevant to patient care.
- Strong communication, advocacy, and interdisciplinary collaboration skills.
- Ability to perform assessments, develop discharge plans, and provide counseling effectively.
- Competency in emergency procedures, confidentiality, and ethical standards.
