The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary setting. They are accountable for program development, quality improvement, problem-solving, and enhancing productivity within a collaborative team environment.
Role OverviewThe Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary setting. They are accountable for program development, quality improvement, problem-solving, and enhancing productivity within a collaborative team environment.
Qualifications & Skills- Minimum education:
High School Diploma (required) or advanced degrees preferred. - Doctorate of Physical Therapy (DPT) required after certain date.
- Experience:
Emphasis on years and type of experience preferred but not mandatory. - Licensure &
Certifications:
Valid Texas Driver’s License, licensed Physical Therapist, Basic Life Support (BLS), Auto Liability Insurance, reliable transportation, and other relevant licensures as specified.
- Perform assessments, develop treatment plans, and deliver therapeutic interventions.
- Contribute to program development and quality improvement initiatives.
- Work within an interdisciplinary team to ensure optimal patient care.
- Licensed Physical Therapist with required licensure and certifications.
- Valid Driver’s License, BLS certification, auto insurance, and dependable transportation.
- Must obtain certain certifications within ninety days of hire.
