Pharmacist, General/Inpatient Job Details

Job Location: Fort Sam Houston, TX

Position Type: Full-Time/Part-Time

Description
  • Work collaboratively with multidisciplinary teams to optimize drug therapy.
  • Receive and review prescriptions for the full range of pharmaceuticals to include controlled drugs on the formulary.
  • Determine type of substance, strength, dosage, and quantity to be dispensed in accordance with accepted practice and pharmacy standing operating procedures (SOPs).
  • Enter prescriptions into outpatient computer system, reviewing profile for allergies, drug/drug or drug/food interactions, and duplicate orders.
  • Coordinate with prescriber to clarify issues or problems with dosing, interactions, or instructions.
  • Obtain the pharmaceutical, prepares package labels, visually inspects the medication in the bottle to be dispensed, and verifies appropriate medication, checks the printed label affixed to the container against the original prescription for correctness.
  • Provide instruction to patients on proper use of medications.
  • Mentors technicians and other pharmacy staff working in/with area of responsibility in order to maintain the accuracy and quality of pharmaceutical services.
  • Provide accurate drug information to health professionals and patients, to ensure safe and appropriate use of medications.
  • Provide medication reconciliation for the patient. Pharmacist may be identified per shift as a Code Responder and will respond to Code Blues during a particular shift.

Full-time and Part-time position Available

Qualifications

STAFF PHARMACIST (INPATIENT PHARMACIST):
One year of pharmacy experience. Previous inpatient experience is preferred.

  • Bachelor s degree in pharmacy. Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Licensure/Registration:
    Current, valid, full and unrestricted license in one of the 50 states and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States.
  • Certification-BLS American Heart Association
