Company OverviewPecan Valley Rehabilitation & Healthcare in San Antonio, TX is a post‑acute care facility offering long‑term, short‑term, and geriatric services. We provide a stable and engaging in‑house therapy team and excellent programs that support our geriatric clientele in their journey to recovery.
Responsibilities- Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence‑based practice to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
- Administer various procedures as part of the rehabilitation plan, including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness and modify services in relation to patient evaluation while making referrals based on outcomes.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Effectively communicate with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summaries of services per regulatory and clinical requirements.
Occupational Therapist license required.
Open to Occupational Therapists at all experience levels. New graduates are welcome.
