Job Summary And QualificationsAs an Advanced Clinical Pharmacist, you will demonstrate proficiency in all pharmacist functions by attending and actively participating in daily multidisciplinary and bedside rounds, completing drug utilization review to promote rational drug therapy, leading and developing clinical services, mentoring and educating staff to ensure accurate and appropriate documentation of clinical activities. You will conduct drug regimen reviews and respond to specific polypharmacy, pharmacokinetic and medication/MAR reconciliation consults.
You will recommend and monitor drug therapy as assigned by the medical staff. Utilize expertise to train, mentor and supervise Pharm
D candidates, pharmacy residents, and technicians. Train and participate in facility wide committees and training programs.
- Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
- Accurately compound and dispense pharmaceuticals.
- Comply with facility policy and procedures and regulatory requirements.
- Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
- Provide clinical consultation and clarification to practitioners. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
- Sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchange and promote rational drug therapy selection.
- Attend and actively participate in daily multidisciplinary and bedside rounds.
- Support the hospital formulary decision process by working with medical staff to promote rational drug therapy and make improvements identified by drug utilization evaluations.
- Provide accurate and timely medication information to healthcare staff and patients.
- Actively participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
- Doctor of Pharmacy (Pharm
D) or Bachelor of Science in Pharmacy.
One of the following is required:
- Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience required; preferably within area of focus.
- Completion of hospital PGY1 / pharmacy general practice residency or fellowship.
- BCPS certification or other Board of Pharmacy Specialty (BPS) certification.
- Current Registered Pharmacist licensed by the Texas State Board of Pharmacy.
- Licensed pharmacist in another state:
Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy.
