The Mental Health Professional II facilitates high quality, patient-centered care by providing supervised mental health services to patients and their families. The Mental Health Professional II performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family, and group therapy. This role primarily supports Methodist's 74-bed secure unit and outpatient clinical patients at Methodist Specialty and Transplant Hospital location.
What you will do in this Role:
Conducts individual, group and family psycho educational and process groups.
Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis.
Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement). Completes assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.
Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge.
Conducts post-discharge follow-up as indicated.
Evaluates re-admissions for gaps in discharge planning processes.
Formulates a plan and implements new strategies.
Develops and maintains a community resource guide including access to public assistance/alternatives.
Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope.
