As a Nurse Practitioner in post-acute care, you will provide in‑person care to patients in skilled nursing and rehabilitation facilities. You will play a critical role in managing both chronic and acute conditions, participating in transitional care planning, and supporting the facility’s quality improvement initiatives.

• Collaborate with Trumen physicians and facility medical directors to manage patient care
• Conduct comprehensive clinical assessments and manage treatment plans
• Participate in transitional care planning to prevent avoidable hospital readmissions
• Reconcile medications and provide patient education
• Foster positive working relationships with facility leadership, clinical staff, and care coordinators
• Serve as a liaison between Trumen and assigned skilled nursing facilities (SNFs)
• Maintain accurate and timely documentation within the EMR
• Be present on‑site during regular business hours (Monday‑Friday), depending on availability

• Active Nurse Practitioner license in the state of Texas
• Minimum of 1 year experience as a Nurse Practitioner, preferably in a post‑acute, SNF, or geriatric care setting
• Ability to work independently while effectively collaborating with the care team
• Strong interpersonal skills and a passion for caring for complex patients
• Reliable transportation within the San Antonio area
