Responsibilities
  • Provide advanced and routine nursing care for residents with complex medical, mental health, and behavioral health issues.
  • Administer medications and perform procedures such as tracheostomy, gastrostomy/jejunostomy care, wound care, and breathing treatments.
  • Collect specimens for laboratory work and conduct health screenings.
  • Document and report treatments and observations accurately in the electronic health record.
  • Instruct and supervise direct support professionals, new LVNs, and other staff on care practices.
  • Assist RNs with protocol implementation and quality improvement initiatives.
  • Respond to emergency situations and follow infection control procedures.
  • Participate in disaster response and continuity of operations as required.
  • Maintain current CPR certification and demonstrate physical capability for CPR.
Essential Job Functions
  • Attend work on a regular basis, following a scheduled shift or rotating schedule as required.
  • Serve in on‑call status on a rotating schedule when operational needs arise.
  • Make rounds and/or home visits as needed and work in other locations within the SSLC.
  • Perform complex nursing procedures in accordance with HHSC, SSLC, and state regulations.
  • Monitor individuals’ weight, diet, feeding, and vital signs; evaluate treatment outcomes.
  • Document and report findings accurately in the medical record.
  • Collaborate with the interdisciplinary team, providing oral and written communication.
  • Lead, monitor, and oversee others in preparing and executing nursing procedures.
  • Train and instruct new LVN staff, providing ongoing feedback and mentorship.
  • Assist with infection control, quality assurance, and therapeutic nursing programs.
  • Perform other duties as assigned, including assistance with medication variance and scheduling.
Knowledge,

Skills and Abilities
  • Knowledge of nursing care standards, procedures, and techniques.
  • Knowledge of emergency procedures and CPR.
  • Knowledge of state and federal laws and ICF‑IID regulations.
  • Reasoning skills to identify and resolve problems.
  • Computer skills for electronic health records.
  • Ability to organize, coordinate, evaluate nursing activities, and interpret policies.
  • Effective communication skills, both oral and written.
  • Strong CPR capability and current certification.
Initial Screening Criteria
  • Graduation from a licensed vocational nurse (LVN) program.
  • At least 6 months of prior LVN employment experience (12 months preferred).
  • Experience working or volunteering with individuals with intellectual or developmental disabilities (preferred).
  • Experience training, teaching, or leading others (preferred).
  • Experience using an electronic health record system (preferred).
Required Registrations, Licensure, or Certifications

Licensed to practice as an LVN in the state of Texas, or a state that recognizes reciprocity through the Nurse Licensure Compact.
