Conviva is a Subsidiary of Humana, seeking a dedicated, compassionate, and cheerful LPN/LVN (Licensed Practice Nurse / Licensed Vocational Nurse), who has a soft spot for our senior members and wants to make a positive impact in their lives. The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region.

Job Functions

* The LPN/LVN assesses patient health problems and needs.

* Takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records.

* May advise patients on health maintenance and disease prevention or provide case management.

* Understands own work area professional concepts/standards, regulations, strategies, and operating standards.

Required Qualifications

* Current state licensure as an LPN or LVN in the state of Texas

* 1+ years' experience as LPN or LVN in an inpatient or outpatient clinic setting

* Hands-on professional Phlebotomy experience

* Experience with Electronic Medical Records

* Experience with HEDIS

* Must be able to work at the Conviva Care clinic located at: 9727 Poteet Jourdanton Freeway, Suite 108, San Antonio, TX 78211

Preferred Qualifications

* IV Certified

* Knowledge of primary care medicine requirements (state specific).

* Bilingual in English and Spanish, with the ability to speak to patients in both languages

Additional Information

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$48,900 - $66,200 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
