Licensed Vocational Nurse LVN Field Nurse FullTime or PRN

• Skilled Nursing Visits – Wound care, medication teaching, vital checks, and patient assessments
• Documentation – Complete visit notes accurately in Axxess EMR (training provided)
• Communication – Report patient changes promptly to RN Case Manager/DON
• Patient Education – Teach patients and families about care plans and medications
• Field Work – Reliable, timely visits to patients’ homes within San Antonio
• (Optional) Telehealth Coordination – Assist with simple remote monitoring follow‑ups when needed

• Active LVN license in Texas
• At least 1 year of nursing experience (home health preferred but NOT required)
• Good communication, professionalism, and reliability
• Comfortable working independently in the field
• Reliable transportation & auto insurance
• Bilingual (Spanish) a plus, not required
