Our organization is seeking a Hospice Registered Nurse Case Manager with excellent knowledge of geriatrics and hospice and palliative care. The successful candidate for this position will be responsible for coordinating patient care plans, developing patient care agreements, communicating with patients and their families, and ensuring that all required evaluations are conducted. We're looking for a professional with strong communication skills who can develop relationships with patients and their families while working collaboratively with other healthcare professionals to ensure adequate end-of-life care.
You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
- Work as part of a team on hospice and palliative care teams who provide comfort and support to patients and their families during a terminal episode of illness.
- Work with nursing staff to coordinate patient care and discharge planning.
- Be skilled in the management of pain and symptom management.
- Knowledge in diagnosing conditions, medication management and communication between care providers.
- Be able to explain medications to patients and families.
- Responsible for all aspects of coordination including, but not limited to, scheduling appropriate follow-up appointments with health care providers, managing medications, referrals and hospital admission.
- Provide assistance in identifying, qualifying and documenting resources.
- Currently licensed Registered Nurse (RN) in Texas
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver's license and Automobile Insurance
- First aid/CPR certification
