Hospice RN Care Manager; Weekend Shifts

Gentiva Hospice is seeking an After hours RN Care Manager to provide high-quality hospice care outside standard hours. You will deliver direct patient care during nights, weekends, and on-call shifts, and maintain clinical oversight for assigned patients.

You will collaborate with physicians and the IDG to update plans of care, educate families, and ensure timely documentation while maintaining infection control standards and compassionate support.
