Summary:
Vighter is recruiting a Registered Nurse (RN), Oncology Nurse Navigator to provide exceptional patient care at Brooke Army Medical Center in San Antonio, TX. The RN provides a range of nursing services within the Oncology scope of practice and ensures that all JCO standards for care are met during performance of patient care.
The Oncology Nurse Navigator is a professional registered nurse with specialized oncology knowledge and clinical experience who provides individualized nursing guidance, care coordination, education and advocacy to patients undergoing diagnosis, treatment, surveillance and follow-up for cancer-related care. The Oncology Nurse Navigator serves as a clinical resource for patients, families, caregivers and members of the interdisciplinary healthcare team.

Principle Accountabilities:
- Serve as a consistent clinical point of contact for assigned oncology patients throughout the applicable phases of cancer care, including diagnostic evaluation, treatment planning, active treatment, surveillance, survivorship, palliative care, and end-of-life transitions.
- Assist patients and their families in navigating complex healthcare systems and oncology treatment pathways, including coordination among medical, radiation, surgical oncology, pathology, radiology, primary care, specialty services, pharmacy, rehabilitation and supportive care.
- Assess patients for treatment-related symptoms, complications, functional changes, psychosocial distress, nutritional concerns, pain, fatigue, treatment tolerance, and other clinically significant findings.
- Conduct comprehensive nursing assessments to identify clinical, educational, psychosocial, logistical, financial, cultural, communication, and other barriers that may interfere with timely access to cancer care.
- Develop and maintain an individualized navigation plan based upon the patient's diagnosis, treatment plan, identified needs, barriers to care, available resources and goals.
- Monitor patient progress through the oncology continuum and proactively identify delays, missed appointments, incomplete diagnostic studies, referral gaps, or other circumstances that may adversely affect continuity or timeliness of care.
- Facilitate appropriate interventions and referrals to address barriers to care.
- Coordinate oncology appointments, consultations, diagnostic procedures, laboratory studies, imaging, biopsies, surgical evaluations, chemotherapy or systemic therapy or radiation services and follow-up appointments as appropriate to the assigned patient population.
- Facilitate communication among physicians, advanced practice providers, nursing personnel, pharmacy, social work, case management, nutrition services, behavioral health, rehabilitation services, palliative care, hospice and other members of the interdisciplinary team.
