Private Duty LVN provides skilled, compassionate nursing care to pediatric and adult patients in their homes.
Key Responsibilities- Deliver direct bedside care and support daily clinical needs.
- Administer medications and perform prescribed treatments.
- Monitor patient condition and report changes to the care team.
- Follow individualized care plans to ensure safe, quality outcomes.
- Collaborate with patients’ families and multidisciplinary staff.
- Document care accurately and maintain required clinical records.
- Maintain infection control practices and promote patient safety.
- Current LVN licensure.
- Strong clinical assessment and medication/treatment administration skills.
- Ability to follow care plans and recognize/report patient changes.
- Excellent communication and teamwork with families and care teams.
- Accurate documentation and adherence to safety standards.
- Compassionate, patient-first approach to home-based care.
