Matrix Medical Network is hiring a Nurse Practitioner (PRN) to conduct comprehensive health assessments at members' residences, including homes and Skilled Nursing Facilities.

• Conduct adult or geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Work with primary care physicians on patient education and follow‑up as necessary.
• Provide services at the member’s place of residence (home or Skilled Nursing Facility).

• Flexibility – Visits are scheduled based on your availability. Assessments run Sunday–Saturday, 8 a.m.–7 p.m.; you can work day, evening, or weekend shifts.
• Competitive Pay – Earnings based on completed assessments. 20–100 visits per month can earn $1,800–$11,500, with per‑assessment rates of $90–$115 depending on state.

• Master’s degree or equivalent experience and completion of NP licensure.
• Current NP licensure in state of practice, including prescription authority or ability to obtain it.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred; new graduates encouraged to apply.
• Valid state driver’s license and proof of adequate automobile insurance coverage.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
