Position: RN-Network Resident Housing Liaison

Essential Functions and Work Responsibilities

The Network Resident Housing Liaison oversees compliance processes for Innov Age PACE/LIFE contracted facility providers. The Network Resident Housing Liaison (RHL) is responsible for compliance activities and processes from initial audit to issue resolution to minimize the risk to Innov Age PACE/LIFE participants in contracted facilities.

Compliance and Risk Management – Contracted Resident Housing Facilities (70%)
  • Assists with network expansion activities by participating in pre-vetting audits for new Resident housing provider contract requests.
  • Proactively gathers information and monitors survey results to identify adverse results.
  • Performs routine audits and prepares reports for Director-Network Resident Housing Oversight and center leadership.
  • Works with PACE/LIFE leadership and Interdisciplinary team to ensure effective partnership and identify new partnership needs.
  • Facilitates resolution of care, quality or collaboration issues and ensures special case monitoring and action planning to increase quality, compliance and mitigate risk.
  • Recommends and assists with implementation of corrective action to improve quality of care and increase collaboration and contractual compliance.
  • Monitors corrective action effectiveness and coordinates follow-up through ongoing account management.
Compliance and Risk Management – PACE/LIFE (30%)
  • Supports Innov Age PACE/LIFE Center leadership to ensure compliance with contractual Resident care housing agreement.
  • Performs desk review of internal quality data, documentation, meeting minutes and functions to ensure regulatory adherence.
  • Prepares requested reports by collecting, analyzing, and summarizing relevant information from investigations and compliance activities.
  • Assists Director-Network Resident Housing Oversight with policy and procedure development.
  • Provides onboarding and orientation of contracted Resident housing facilities in collaboration with Center leadership and Network.
  • Performs other related duties as assigned.
Travel Requirements (60%)
  • Travels between local Innov Age Centers.
  • Travels to targeted network development and contracted Resident Housing facilities.
  • Travels out of state as needed.
Qualifications
  • Bachelor’s degree in an appropriate clinical discipline or public health. RN/LPN or clinical degree/license preferred.
  • Valid state issued driver’s license, personal transportation, safe driving record and auto insurance required.
  • A minimum of 3 years of experience with current and relevant knowledge of applicable state and federal regulations for Long-Term Care and Assisted Living facilities.
  • Registered Nursing License preferred.
  • Current knowledge of P.A.C.E. Regulations preferred.
  • Knowledge of interviewing and investigation techniques and implementation of corrective action plans.
  • Previous experience working in or closely with a Long-Term Care or Assisted Living Resident housing facility.
  • Skilled at having difficult and direct conversations, identifying problems, rapport and relationship building, analyzing and assessing situations.
  • Communicates effectively both orally and in writing; able to act independently; interprets and explains complex rules and regulations.
  • A minimum of one year working with the frail or elderly required.
