We are seeking skilled and compassionate Registered Nurses (R.N.s) to join our growing team across the Bay Area. The ideal candidate is clinically strong, patient-centered, collaborative, and committed to delivering care that is thorough, responsive, and never rushed. As an Alpha Care RN, you will play a critical role in helping patients recover safely at home while coordinating care across physicians, therapists, caregivers, and support staff.

This role includes completing OASIS documentation, developing individualized care plans, supervising LVNs and HHAs, and helping patients and families navigate care confidently in the home environment. As an Alpha Care RN, your role is to deliver more than task-based care. You are responsible for creating a patient experience built on clinical excellence, communication, dignity, and trust.

Responsibilities include:

• Conductinitialhomecarevisitsandregularlyreassesspatientneedsandprogress
• Developandimplementindividualizedcareplansbasedonphysicianorders
• CompleteOASISassessmentsforadmissions,transfers,resumptionsofcare,re-certifications,and discharges
• Observe,assess,anddocumentpatientsymptoms,responses,and progress
• Educatepatientsandcaregiversondiseasemanagement,medications,treatment plans,andhomehealthcaretechniques
• Coordinatepatientservicesandschedulingacrossthecareteam
• SuperviseandsupportLVNsandHHAsinthedeliveryofqualitypatientcare
• Communicateproactivelyregardingchangesinpatientconditionwithphysicians,Clinical Managers,therapists,andcasemanagers
• Facilitatecollaborativecaseconferencestoensurecontinuityandqualityofcare
• Maintain timely,accurate,andcompliantclinicaldocumentation
• Dischargepatientsfromskillednursingserviceswhendischargecriteriaaremet
• Participateincontinuingeducation,in-service training,andprofessionaldevelopmentprograms
• Complywithallagencypolicies,procedures,Medicare,and Medicaid regulations
