OverviewThis role is based in an Assisted Living, Independent Living, and Memory Care setting. The Occupational Therapist provides patient care, including screening and evaluation, developing and implementing treatment plans, supervising assistants and rehab technicians, and managing case responsibilities while maintaining compliance with state and federal regulations and Reliant Rehabilitation guidelines.
Responsibilities- Optimise the functionality and skills of patients by providing, directing, and/or overseeing occupational therapy.
- Screen residents, as appropriate.
- Evaluate and treat patients who may benefit from therapy; referred by state‑approved referral sources.
- Comply with state and federal guidelines regarding accurate and timely billing and documentation in the medical record.
- Establish and maintain effective interactions with patients, the interdisciplinary team, and other clientele.
- Refer patients for treatment to Certified Occupational Therapy Assistants (COTA) throughout the cluster.
- Provide consultation and clinical supervision to COTAs throughout the cluster.
- Maintain availability to answer questions from COTAs.
- Directly supervise COTAs and Rehab Techs in skilled tasks during group and individual treatment, promoting efficient resource utilisation and clinical skill development.
- Build a strong occupational therapy program by engaging in the following activities:
- Actively integrating the OT’s role into the community and interdisciplinary treatment team.
- Contributing to community patient care, utilisation review, case management, administrative staff/department head, and family conference activities and communications.
- Establishing and maintaining good working relationships with other healthcare professionals and colleagues in the community.
- Making appropriate referrals to other members of the rehabilitation team.
- Developing and implementing new programs/techniques to enhance rehabilitation services.
- Adhering to Reliant Rehabilitation and client‑specific policies and procedures related to environmental safety, orderliness, risk management, infection control, and emergency preparedness.
- Adhering to the requirements of patient rights and privacy.
- Enhance clinical expertise, professional, and management skills through interaction with managers, therapists, and other professional staff, self‑study, and other continuing education activities.
- Maintain a valid state licence where applicable.
- Understand relevant payment models related to billing and treatment guidelines, and provide quality care with reasonable goals and outcomes within the guidelines.
- Be compliant with infection control procedures and environmental safety protocols within a community.
- Perform patient lifting using appropriate biomechanical techniques frequently throughout the shift; must be capable of lifting 50 pounds of dead weight alone.
- Maintain confidentiality of Reliant Rehabilitation, community, and patient information.
- Conduct oneself in an ethical, legal, and responsible manner at all times.
- Adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct.
- Travel between communities during scheduled hours.
- Work varying hours and shifts, including weekends and holidays.
- Attend all mandatory meetings, trainings, and assignments as delegated.
- Perform other duties and responsibilities as assigned.
- Effective clinical problem‑solving skills: identify and prioritise issues, develop sound action plans, execute plans on time, and ensure appropriate results.
- Strong team player.

- Ability to build strong relationships with a diverse population.
- Ability to read, write, speak, and comprehend in English.
- Ability to work in a fast‑paced, productive work environment.
- Graduate of an ACOTE‑accredited school of occupational therapy.
- Proficient knowledge of Microsoft Office (Outlook, Word, Excel, PowerPoint, etc.).
- Basic internet navigation skills.
- Valid certification from the National Board for Certification of Occupational Therapy (NBCOT).
- Valid Occupational Therapist state licence (where applicable).
