Job Purpose

The Resident Services Coordinator – Supportive Housing, works independently and collaboratively with a multidisciplinary team to ensure high-quality service delivery to older adults aged 62 and over. This hybrid role combines individualized case management with onsite resident engagement, offering resource navigation, advocacy, and enrichment activities to promote healthful, stable living for residents in the community.

About Serving Seniors

Established in 1970, Serving Seniors helps San Diego County’s low-income seniors overcome poverty through whole-person, wraparound support including meals, housing, health and social services, and enrichment activities. We work at numerous sites across the county and in the homes of hundreds of homebound seniors. Our flagship Gary & Mary West Senior Wellness Center in downtown San Diego is unlike any other senior center in the nation.

Bringing together a range of supportive services under one roof, the Senior Wellness Center operates 365 days a year and offers two floors of integrated care, including health and social services, housing navigation, enrichment activities, lifelong learning, a Cyber Café and linkage to over 25 onsite direct service partners, including a senior dental center. Last fiscal year, we provided over 1,400,000 meals and coordinated services to 6,800 older adults.

Requirements

Job Duties and Responsibilities Client Support & Resource Navigation
  • Provide individualized case management and connect clients to both in-house and external community resources, including health, mental health, and social services.
Community Engagement & Outreach
  • Conduct outreach within community to inform and educate older adults on relevant topics and available services.
  • Plan, coordinate, and implement onsite activities such as cultural celebrations, social events, skill-building workshops, and holiday gatherings.
  • Facilitate life skills workshops in partnership with community organizations.
  • Responsible for outreaching to residents on a regular basis to increase and maintain participation in programs.
Program Coordination
  • Collaborate with the Volunteers and Activities team to create monthly calendars of activities and events for Ramona residents.
  • Maintain attendance logs and bulletin boards.
  • Create and distribute flyers for events and activities.
Partnership Development
  • Build and maintain collaborative relationships with community partners to bring resources and programs onsite.
Housing Support
  • Interface with property management to assist with conflict resolution, tenancy issues, and eviction prevention services.
Assessment & Planning
  • Conduct Universal Senior Assessments and develop care plans to address clients’ unmet needs, including referrals to community-based agencies.
