As an RN you will provide optimum care to homebound patients throughout their homes, performing activities such as medical necessity review, initial assessments, continued stay review, discharge planning, and other duties. You will utilize all elements of the nursing process, assess, and evaluate patient/client status, and provide professional nursing care.

This position uses a pay‑per‑visit compensation model. Pay per visit ranges from $50 to $160 commensurate with experience and depending on type of visit.

As an RN Case Manager you will also: complete and submit accurate electronic clinical notes, administer medications and treatments as prescribed, assist patients with activities of daily living, teach self‑care techniques, and maintain a safe environment for the patient.

Qualifications:

• Graduate of an approved school of professional nursing, BSN preferred
• Currently licensed to practice as an RN in the state of practice
• Clinical knowledge of various disease processes
• One year of acute nursing experience in medical, surgical, and/or critical care (home health experience preferred)
• Excellent observation, good nursing judgment, and strong oral and written communication skills
• Self‑directing, flexible, and cooperative
