Field LVN
- Per Diem

Join our team as a Full‑time Licensed Vocational Nurse (LVN) in San Joaquin County. Lorian Health is a home health agency that is thoughtful, generous, and family‑oriented, placing focus on taking the best care of our patients and our employees.

Qualifications
  • LVN/LPN licensed to practice in the state of practice and in good standing.
  • Graduate of an approved school of practical/vocational nursing.
  • One (1) year experience as a professional nurse in an acute setting.
  • Possesses and maintains current CPR certification (BLS required).
  • Possession of a valid driver’s license.
  • Ownership of an automobile insured according to Lorian requirements.
  • Ability to read and follow written instructions and document care given in the patient’s electronic medical record.
  • Excellent observation, good nursing judgment, and good oral and written communication skills.
  • Self‑directing and able to work with little direct supervision.
  • Ability to work with adults (18 years and older), demonstrating tact and patience.
  • Flexible and cooperative in fulfilling role obligations.
  • Participation in in‑service education programs and clinical meetings.
  • Adherence to infection‑control policies and protocols.
  • Maintenance of confidential patient records in accordance with HIPAA and other applicable state laws.
Responsibilities
  • Travel to patients’ homes throughout the day and provide direct nursing care as prescribed by the physician, under the direction and supervision of a Registered Nurse.
  • Observe patients/clients regularly for changes in condition, discuss changes with the patient/clients or family/caregiver, and document and report changes as necessary to the attending physician.
  • Educate and instruct patients, families, and other caregivers regarding patient care.
  • Assume additional job‑related tasks beyond those listed as required.
