Qualifications
  • Enthusiasm for providing the best care to patients and families
  • RN license in good standing in CA
  • Preferred: 1 year of hospice or home health experience
  • Valid driver’s license
  • Current CPR certification
  • Excellent observation, verbal, and written communication skills
Daily Responsibilities
  • Create and initiate care plans based on patient assessments
  • Spend quality time with patients and families to support their needs
  • Conduct initial and ongoing assessments of patient and family needs
  • Record vital signs and health information
  • Administer medications and treatments as prescribed
  • Provide health care instructions to patients
  • Document symptom management and outcomes
  • Maintain accurate clinical notes
  • Perform additional duties as assigned
