- RN license in good standing in CA
- Preferred: 1 year of hospice or home health experience
- Valid driver’s license
- Current CPR certification
- Excellent observation, verbal, and written communication skills
- Create and initiate care plans based on patient assessments
- Spend quality time with patients and families to support their needs
- Conduct initial and ongoing assessments of patient and family needs
- Record vital signs and health information
- Administer medications and treatments as prescribed
- Provide health care instructions to patients
- Document symptom management and outcomes
- Maintain accurate clinical notes
- Perform additional duties as assigned
