The Registered Nurse plans, organizes and directs home health and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Responsibilities- Initial, comprehensive, and ongoing assessment of patient and family to determine home health needs; complete physical assessment and history of current and previous illness(es).
- Develop a care plan that establishes goals based on health assessment data and nursing diagnosis, incorporating palliative nursing actions.
- Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician.
- Counsel patient and family in meeting nursing and related needs; provide health care instructions as appropriate.
- Assist patient with activities of daily living and facilitate self‑sufficiency and optional comfort care.
- Act as case manager when assigned by clinical or nursing supervisor; coordinate care for assigned caseload.
- Identify discharge planning needs and implement prior to discharge.
- Maintain accurate clinical notes; record pain/symptom management changes/outcomes.
- Communicate with physician regarding patient needs; obtain and execute orders.
- Teach patient and caregivers self‑care techniques; provide medication, diet and other instructions; collaborate with team for continuity of care.
- Provide and maintain a safe environment; attend interdisciplinary group meetings.
- Registered nurse with current licensure to practice professional nursing in the state.
- Graduate of National League for Nursing accredited school of nursing.
- Maintain current CPR certification.
- Licensed driver with an automobile insured per state/organization requirements and in good working order.
- Minimum one year of experience as professional nurse within last three years OR baccalaureate degree in nursing from NLU program and current RN license; preference for two years experience, at least one in public health, home care, home health, or hospice nursing.
- Self‑directed, able to work with minimal supervision; responsible for supervising home health aides.
- Excellent observation, problem solving, verbal and written communication, nursing skills per competency checklist.
- Ability to organize and prioritize workload independently.
- Physical ability to walk, stand, lift, transfer patients, handle supplies; visual acuity and hearing sufficient for required nursing skills.
Job Type: Full‑time
Pay: $80.00 - $100.00 per hour
