Job Description Summary
An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring patient safety and continuity of care under a physician’s plan.
Job Responsibilities- Initiate, develop, and implement intermediate nursing plans of care treatments, evaluating patient progress towards goals. Provide direct patient care, administer medications, and educate patients and families or caregivers.
- Assess patient needs and physical status at each skilled visit, re-evaluating them throughout treatment. Obtain and clarify physician orders, informing physicians promptly of significant changes in condition.
- Provide training and mentorship to Infusion Nurse I’s to ensure safety and compliance.
- Teach patients and families using a patient‑centered approach to achieve independence with therapy and care needs.
- Communicate effectively and timely with the clinical team, medical providers, patients, and caregivers to facilitate continuity of care. Prepare and submit real‑time documentation to the nursing department within billing timelines.
- Coordinate discharge planning, prepare discharge summaries, and report patient care needs and progress when transferring care.
- Demonstrate compliance with agency operations, policies, professional standards, local, state, federal regulations, and accreditation standards.
- Maximize work efficiency through computer use and automation technologies for plan verification and schedule adherence.
- Respond promptly and appropriately to patient requests and initiate emergency procedures as necessary. 법
- Accept accountability for own practice through ethical conduct and adherence to established programs.
- Observe legal, ethical guidelines, HIPAA regulations, and confidentiality of patient and proprietary information.
- Communicate knowledge about services and financial obligations to patients.
- Complete required training, in‑services, and continuing education. Provide oversight and coordination of paraprofessionals, and act as Supervisor when appropriate.
- Participate in multidisciplinary team conferences; provide precepting, training, and mentoring to nurses for orientation and supervisory activities.
- Participate in nursing department on‑call responsibilities.
- Perform other related duties as directed by supervisor.
- Insert PIC ощущm/midlines, troubleshoot or close obstructed central line catheters.
- Monitors PICC/Midline, educate on prevention of line infection, and perform routine care.

BasicEducation and Experience
- Active and unrestricted RN license in the state of practice.
- Minimum 1 year of nursing experience.
- Current CPR certification required.
- Competency in patient care standards and infusion skill sets required for safe service delivery and agency programs.
- Valid driver’s license and access to a vehicle with liability insurance for business travel.
- Basic computer knowledge and proficiency in nursing technology tools for documentation and communication.
- Ability vadylift up to 50 pounds (maximum 75 pounds). Frequent light lifting up to 10 pounds and occasional lifting up to 20 pounds.
- Physical demands include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overdose, grasping, pushing, pulling, lifting, carrying.
- Visible skills and fine motor ability required, including manual dexterity and visual acuity.
- Exposure to blood, body fluids, hazardous substances, sharps, needles, and unpredictable home environments.
- Safety requirements include closed‑toe shoes, facemask, goggles, gown, gloves, and effective communication device.
- Willing to travel up to 100% of the time for business purposes.
- Bachelor of Science in Nursing (preferred).
