** About the Role
** As an
** On Call*
* ** Licensed Vocational Nurse**, you will provide direct patient care under the supervision of a Registered Nurse. You’ll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient.
*
* Schedule:

** Nights and weekends
*
* Key Responsibilities:

*** Implement nursing care based on comprehensive assessments and care plans
* Accurately document patient services to reflect the plan of care
* Educate patients and families on diagnoses and self-care plans
* Coordinate hospice services with the interdisciplinary team
* Use equipment and supplies effectively and efficiently
* Provide pain and symptom management, evaluating patient response
* Participate in quality improvement and professional development activities
*
* Qualifications:

*** Valid California LVN license
* Minimum 2 years of hospice experience
* Experience with Home Care Home Base (HCHB) preferred
* Current CPR certification
* Valid driver’s license and insured, reliable vehicle
