The Admissions RN at ANCHOR Hospice will present our philosophy and services to patients and families to support admission of appropriate patients. You will participate in information gathering, analysis, education and form completion to ensure accurate admissions.
You will evaluate patient suitability, obtain admission agreement, coordinate with LPNs for orders, and build strong referral relationships while maintaining clinical competence and internal collaboration with admissions and care
